  • 摘要: 终板缺损是椎间盘退变及退变性腰痛的关键病理基础,其通过破坏椎间盘营养通路、改变局部生物力学环境及诱发炎症反应引发疼痛。多项横断面及纵向影像学研究表明,终板缺损评分与椎间盘高度指数呈显著负相关,与Pfirrmann分级呈正相关,并增加Modic改变的发生风险,提示其在退变性腰痛中具有重要作用。文章总结了针对终板缺损的干预策略,重点讨论了生物力学干预、抗炎药物、再生医学和影像学引导下的精准治疗,并探讨了中西医结合治疗终板缺损相关腰痛的潜力。未来研究应聚焦于整合影像学、生物力学及分子生物学证据,进一步明确不同类型终板缺损在疾病进展中的作用,并探索基于中西医结合的治疗模式,以推动退变性腰痛的个性化治疗。

     

    Abstract: Endplate defects are a key pathological basis for intervertebral disc degeneration and degenerative low back pain. They cause pain by disrupting the nutritional pathway of the intervertebral disc, altering the local biomechanical environment, and triggering inflammatory responses. Imaging studies show that endplate defects are significantly correlated with intervertebral disc height, Pfirrmann grade, and the incidence of Modic changes, indicating their important role in degenerative low back pain. This article summarizes intervention strategies for endplate defects, focusing on biomechanical interventions, anti-inflammatory drugs, regenerative medicine, and image-guided precision treatments. Additionally, it explores the potential of combining traditional Chinese and Western medicine in treating low back pain associated with endplate defects. Future research should focus on integrating imaging, biomechanics, and molecular biology evidence to further clarify the role of different types of endplate defects in disease progression and explore treatment models based on integrative medicine to promote personalized treatment for degenerative low back pain.
