动静脉内瘘闭塞96 h利用血管鞘超声引导下溶栓成功一例

Application of vascular sheath ultrasound-guided thrombolysis in treatment of arteriovenous fistula occlusion for 96 h: a case report

  • 摘要: 自体动静脉内瘘是维持性血液透析患者的生命线,内瘘血栓形成、闭塞是其最常见的并发症之一。该文报道一例动静脉内瘘闭塞96 h利用血管鞘超声引导下溶栓成功患者,提示利用血管鞘超声引导下移动式脉冲式注射溶栓法操作简单、创伤小、溶栓精准且可抽吸部分血栓、并发症少、安全有效且内瘘再通成功。该溶栓方式可使尿激酶直接作用于血栓部位,安全有效,且超声实时引导下直观形象、费用小,能有效延长动静脉内瘘使用时间,减少血液透析患者的痛苦、节约费用,值得尝试。

     

    Abstract: Autologous arteriovenous fistula is one of the most important measures to maintain the lifeblood of hemodialysis patients.The incidence and occlusion of internal fistula are one of the most common complications. In this article, vascular sheath ultrasound-guided thrombolysis was successfully performed in the treatment of the arteriovenous fistula occlusion for 96 h. The portable pulsed thrombolysis guided under vascular sheath ultrasound has the advantages of simple operation, mild trauma, accurate thrombolysis, aspiration of partial thrombus, few complications, safe, efficacious and success. This approach is safe and efficacious, which enables urokinase to directly act on the thrombus site. In addition, it is objective under the ultrasound guidance, effectively prolongs the use time of arteriovenous fistula, alleviates the pain and reduces the cost of hemodialysis patients, which is worthy of widespread application in clinical practice.
