宫腔镜操作系统冷刀微剪分离术在宫腔粘连患者临床治疗中的可行性分析

Analysis of feasibility of HEOS cold knife hysteroscopic micro-shear separation in clinical treatment of patients with intrauterine adhesion

  • 摘要: 目的 探讨宫腔镜操作系统(HEOS)冷刀微剪分离术在宫腔粘连患者中的临床疗效及安全性。方法 收集200例宫腔粘连患者,根据其治疗方式分为HEOS冷刀微剪分离术治疗组(观察组)和传统宫腔镜下电切分离术治疗组(对照组),每组各100例,比较2组患者的手术指标、疗效与术后并发症。结果 观察组的手术时间短于对照组,其术中出血量、膨宫液吸收量均少于对照组(P均< 0.05)。观察组与对照组的临床疗效比较差异有统计学意义(P < 0.05),观察组痊愈率为61%,高于对照组的45% (P < 0.05)。观察组患者中,发生术后出血1例、感染2例、盆腔粘连1例,并发症发生率为4%;对照组患者中,发生术后出血5例、感染2例、异位妊娠3例、盆腔粘连5例,并发症发生率为15%。观察组的并发症发生率低于对照组(P < 0.05)。结论 采用HEOS冷刀宫腔镜微剪分离术治疗宫腔粘连患者,疗效良好,安全性高。

     

    Abstract: Objective To evaluate the clinical efficacy and safety of hysteroscopy endo-operative system (HEOS) cold knife hysteroscopic micro-shear separation in the clinical treatment of patients with intrauterine adhesion. Methods 200 patients with intrauterine adhesion were enrolled and divided into the observation (n = 100) and control groups (n = 100) according to different treatment methods. In the control group, patients were treated with traditional hysteroscopic electrosurgical separation, and those in the observation group received HEOS cold knife hysteroscopic micro-shear separation. The surgical indexes, clinical efficacy and postoperative complications were statistically compared between two groups. Results In the observation group, the operation time was significantly shorter, and the amount of intraoperative bleeding and absorption of uterine distention fluid were significantly less than those in the control group (all P < 0.05). There was a significant difference in clinical efficacy between two groups (P < 0.05). In the observation group, the cure rate was 61%, higher than 45% in the control group (P < 0.05). In the observation group, postoperative bleeding occurred in 1 case, 2 cases of infection and 1 case of pelvic adhesion. The incidence of postoperative complications was 4%. In the control group, postoperative bleeding occurred in 5 cases, 2 cases of infection, 3 cases of ectopic pregnancy and 5 cases of pelvic adhesion. The incidence of postoperative complications was 15%, which was significantly higher than 4% in the observation group (P <0.05). Conclusion HEOS cold knife hysteroscopic micro-shear separation is an efficacious and safe treatment for patients with intrauterine adhesion.
